Coronavirus (COVID-19) Home Support

With the patient, Our Health Coaches will devise a personalised support plan to help the patient to:

• Address specific issues in their health which may increase their susceptibility to Coronavirus.
• Provide support, advice, and reassurance to help them manage your health at this time.
• Identify the best NHS resources to support their specific health needs at home.
• Help them better understand and manage their specific conditions, medications and treatments.
• Coordinate and communicate with other health and social care services where needed.
• Gain knowledge, skills, and confidence to improve their health and quality of life.
The health coaching sessions take place by telephone or video link at a time that suits the patient. The coaching process runs for between 2 and 3 months, depending on their needs. Of course, if they change their mind, they are free to leave the coaching process at any time. The Service is not appropriate for this these patients:
• Patient has cognitive impairment
• Profound hearing loss
• Acute mental health diagnosis
• In mental health crisis
• Homeless
• Paediatrics (under 18’s)
• Patient is registered outside of Kent and Medway CCG
• Palliative/terminal state
